Please complete the form below, providing us with accurate information about your issue or fault so we can try and resolve it for you. This form must be completed by the account that purchased the product. We can deal only with the account holder when communicating and inspecting any goods that they deem to be faulty.
The information you provide us will be looked into by our complaints team. They will make the final decision on whether you need to send your product/s back to us for testing.
The form must be fully completed and truthful for your issue to be looked into. Any form that is not fully completed, or found to be false will not be looked at.
If we have instructed you to return your hair back to us for testing, you must return all of the hair. If we do not receive the full amount of hair to test, we will send it back to the sender and no further action will be taken.
